Assessment & Planning
Tailored Support for Every Individual
Our process begins with a thorough Assessment & Planning phase, where we evaluate each person’s unique situation and develop an in-depth care plan that addresses their specific needs. Whether the individual is managing a chronic health condition, behavioral health challenges, or simply needs support with day-to-day activities, we work to assess overall well-being.
We conduct comprehensive assessments of physical health, mobility, and the ability to perform Activities of Daily Living (ADLs). For those needing extra assistance, we evaluate the home environment, ensuring it is safe and accessible by recommending necessary modifications or equipment.
Our Access Agents also evaluate the social and emotional well-being of individuals, identifying risks of isolation and ensuring appropriate emotional and social support is integrated into their care plan. Through this process, we create a comprehensive roadmap that considers all aspects of the individual’s life and well-being.


Access & Linkage
Connecting Individuals with Vital Resources
Once the care plan is in place, we move into the Access & Linkage phase, where our Access Agents serve as the bridge between individuals and the full spectrum of essential services from healthcare and wellness to social, emotional, financial, and legal support.
We facilitate connections to healthcare providers, ensuring each person can access preventive care, treatment for chronic conditions, and specialized services like physical therapy or nutrition counseling. For those managing mental health concerns, we help them find the right therapists, psychiatrists, or support groups, ensuring they receive the behavioral health services they need to maintain their mental and emotional well-being.
Extending to Everyday Needs
Our services extend to practical everyday needs as well. We help you access home health aides or personal care attendants to assist with ADLs and connect with providers of Durable Medical Equipment (DME), such as walkers, hearing aids, or hospital beds, ensuring these items are safe, durable, and tailored to your needs.
Financial Planning
Our Access Agents coordinate with the top financial planners and legal experts, assisting with estate planning, long-term care insurance, and ongoing financial management. We help navigate public benefits like Medicare, Medicaid, and Social Security, making sure financial needs are met and future care is planned.
Critical Housing Support
We also provide critical support in housing and safety, connecting individuals to affordable, accessible housing options and ensuring their homes are equipped with essential safety features. For those in housing transition, we assist with housing placement and security.
Support & Coordination

Our Access Agents ensure ongoing coordination between healthcare providers, helping individuals adhere to treatment plans and stay on top of their medical appointments. We monitor the use of durable medical equipment, ensuring it remains safe and effective, and we check in regularly with caregivers and family members to adjust care as needed.
We also take the lead in coordinating behavioral health services, helping maintain access to therapy and psychiatric care. If crises arise, we act swiftly to connect you with emergency services or other urgent care solutions.
