The need for coordinated care is growing fast. In Santa Clara County, nearly 1 in 5 residents will be over age 65 by 2030, and over 40% of adults 50+ say they’re concerned about their ability to age in place. At the same time, local hospitals, social services, and families are overwhelmed, especially in cities like San Jose, Gilroy, Milpitas, and Mountain View, where economic pressure collides with caregiving demands.

According to the California Department of Aging, over 70% of long-term care is provided informally by family members, many of whom are juggling full-time work and responsibilities of their own.
